What documents do I need for a tourist visa?

A correctly completed visa application form (downloadable from the embassy's website) with three recently taken color passport photos
At least 6 months valid with two blank pages, original passport together with copies of the information page (citizens other than Dutch, Polish, Croatian and Bosnian citizens should submit a residence permit or proof of residency issued by the Dutch authorities)
Flight details/booking for a visit to Bangladesh (a confirmed ticket is not required)
All applicants who are under 18 years of age must provide a written consent from their parents with their copies of passports/IDs along with their birth certificate/municipal certificate (extract).
